Job Description Job Title: Social Worker II (LMSW / LCSW) –
Hospital Care Management Location: Oklahoma City, Oklahoma, United
States Pay Rate: $40 – $45/hr. on W2 (Depending on Experience)
Employment Type: 3-Month Contract with Possible Extension
Responsibilities Provide social services and support to patients
and their families within assigned medical services. Conduct
psychosocial assessments to evaluate patient and family needs.
Perform discharge planning evaluations and coordinate care
transitions. Counsel patients and families regarding illness,
treatment, emotional stress, and social or financial concerns.
Provide crisis intervention services for patients and families.
Identify high-risk social cases, including potential abuse or
neglect, and document findings appropriately. Act as a liaison
between patients, families, healthcare staff, and community
agencies. Assist with post-hospital placement, rehabilitation
referrals, or community support services. Advocate for patients
regarding informed decision-making, ethical care issues, and
end-of-life planning. Support patients and families as they adjust
to illness, treatment changes, or functional limitations. Maintain
documentation and records related to referrals, extended stays, and
social service activities. Qualifications Master’s Degree in Social
Work (MSW) from an accredited institution. LMSW or LCSW license
(Oklahoma) required at the time of submission. Previous hospital or
clinical social work experience preferred. Required Skills
Compassionate and empathetic approach to patient care. Strong
communication and interpersonal skills. Ability to work
collaboratively within a healthcare team. Proficient in crisis
intervention techniques. Preferred Skills Experience with
electronic health records (EHR). Knowledge of community resources
and support services.
