Provides primary triage and referral for persons seeking mental
health and substance abuse services. Creates a welcoming and
engaging environment as the first point of entry for NorthCare and
initial engagement in services at NorthCare. Provides
recovery-focused case management, crisis diversion and rehab
services to meet the needs of those seeking services, or assigned
to case load.
DUTIES AND RESPONSIBILITIES (includes but not limited too):
Provides 55% of time in direct consumer service. Part time
limited to 29 hours per week.
Provides services that are welcoming, engaging and accessible.
These services are recovery focused, strengths based, culturally
competent, consumer centered, and co-occurring capable. Works as
part of treatment team to meet the needs of the consumer in a
timely and thorough manner. Interacts with consumers in a
respectful, genuine and professional manner.
Provides initial triage and assessment by phone or in person to
persons seeking services which includes gathering of information
about the symptoms, behaviors, functioning, history, needs and
preferences to assist in decision-making process. Facilitates
referral of consumers into the appropriate agency program(s) or to
another community provider. Obtains the necessary releases and
consents and advises the consumers about their rights and
responsibilities. Consults with other program staff, as needed, to
facilitate referrals. Completes fee agreements and necessary
documentation associated with the fee agreement paperwork. Becomes
knowledgeable of types of insurances, verification and different
contracts within the agency.
Schedules intake and medication clinic appointments for
consumers referred into the agency. Makes follow up calls to engage
the consumer and facilitate their recovery process. Completes
discharge paperwork as required by agency policies.
Provides case management services for consumers, assisting them
in obtaining needed benefits and resources, including, but not
limited to, housing, employment, medical, recreation, social and
family supports, nutritional information and medication.
Establishes linkage with inpatient treatment teams and provides
support services to consumers who are hospitalized and being
discharged from hospitalizations. Must be knowledgeable of
community resources and maintain positive working relationships
with other agencies.
Contacts all consumers on case load at least once a month to
monitor progress toward goals and objectives, determine needed
resources and assistance, and to provide support and encouragement
in the recovery process.
Provides transportation as required to various sites in the

Facilitates development and/or maintenance of relationships with
family, friends, neighbors and significant others in the

Participates in weekly program meeting. Initiates and/or
participates in treatment team staffing meetings as appropriate.
Maintains appropriate staffing documentation.
Assists consumers with the development of their treatment and
recovery plan, and coordinates involvement in services.
Under the close supervision of the Supervisor and other
management staff and/or with the assistance of an LBHP Clinician,
provides crisis management services. This may include face-to-face
or telephone interviewing of the consumer; assessment of a
consumers lethality to self or others; assessment of a consumers
available support systems; and assistance in arranging for
appropriate intervention, including hospitalization. In the cases
where a potential for harm exists, will consult with management
staff or available therapist to develop a safety plan with the

POSITION REQUIREMENTS (skills, knowledge, ability):
Candidate/employee must possess bachelors degree in psychology,
social work or other related human services field. Must obtain
ODMHSAS Case Management certification within 90 days of hire, if
not currently certified.
Must demonstrate good interpersonal skills and the ability to
effectively communicate verbally with English-speaking consumers.
Must display proficiency in the MS Word processing software and
intermediate general computer skills. Local travel is required.
Must possess and provide verification of current automobile
liability insurance including uninsured motorist coverage. Employee
must be able to move about the facility freely and be able to sit
for extended periods. Must have current (within last year) TB test,
or obtain within first 30 days of employment. Must possess and
maintain current certification in CPR, First Aid, and CAPE; or
obtain within 30 days of employment with NorthCare if required by
Clinical Director. Employee must possess or obtain Core Level
Co-Occurring Training and Trauma Informed Training via NetSmart
University within 30 days of employment with NorthCare.
