Job Description Certification Details - BLS Job Details -
Provide social services to patients and their families at OU
Health. - Conduct discharge planning evaluations of patients and/or
family members. - Make psychosocial assessments and communicate
findings to hospital staff. - Counsel patients and others regarding
diseases, treatments, social or financial situations, emotional
stress, and other problems. - Conduct high social risk case
findings and provide crisis counseling regarding child abuse,
neglect, physical abuse, etc. - Provide crisis intervention
services for patients and families. - Act as a liaison between
patients/families and hospital staff and other agencies. - Evaluate
effectiveness of services and adjust patient care as needed. -
Assist in discharge planning and referrals to community services. -
Assist with post hospital placement or transfer to post-acute care
facilities. - Assist patients and families with adjustments to
illness, treatment, or changes in functional status. - Utilize
specialized interviewing and communication skills with children. -
Advocate for patients on issues related to informed decision
making, end-of-life care, goal setting, and ethical dilemmas. - May
prepare statistics concerning referrals and extended stays. - May
supervise students or newly hired employees. - Perform other duties
as assigned. Job Requirements - Hospital experience preferred. -
Knowledge of principles and practices of social and psychological
casework. - Knowledge of social and welfare resources available in
the community and hospital. - Knowledge of current social and
economic conditions, trends, and problems. - Strong analytical
skills to interpret situations and determine appropriate actions. -
Ability to explain technical data in understandable terms to
patients, families, and hospital staff. - Good interpersonal
skills. Additional Details - Experience: Hospital experience
preferred. - Knowledge of social and psychological casework
principles and practices. - Analytical skills to interpret
