Care Nurse collaborates closely with our Primary Care Providers to
deliver safe, timely, effective, efficient, equitable, and
patient-centered care in a structured manner. This role involves
partnering with patients, healthcare providers, and other members
of the clinical team to enhance quality outcomes for a designated
patient panel. All responsibilities will be executed in alignment

Essential Qualifications: Education: Diploma in Associate of
Applied Science in Registered Nursing Licensure: Current RN
licensure Preferred Qualifications: Education: Bachelor of Science
