Job Description:

Job Summary:Mercy Hospital Oklahoma City is seeking a skilled RN Home Health Specialist to join our dedicated Home Health team. As a Home Health RN, you will provide medical care directly to patients in their own homes, focusing on post-hospitalization recovery, rehabilitation, and disease management.Responsibilities:

• Travel to patient homes to provide wound care, post-operative follow-ups, and therapy sessions
• Develop and implement individualized care plans to promote patient independence and smooth recovery
• Collaborate with patients, families, and healthcare teams to ensure optimal care and transition from hospital to home
• Provide education and support to patients and families on disease management and self-care
• Document patient progress and outcomes in accordance with hospital policies and regulationsRequirements:
  • Nursing Degree from an accredited school
  • Oklahoma RN licensure
  • Basic Life Support certification through the American Heart Association
  • At least 1 year of Home Health/Hospice experience as a RN
  • Preferred certifications: ACLSWhat We Offer:
