Providence is currently seeking a highly motivated Assistant
Director of Nursing / Clinical Coordinator.
-Qualified candidates must be a licensed nurse in the state of
Oklahoma (LPN or RN).
Responsibilities and desired skills include:
Clinical:
Work closely with DON to coordinate care
Manage scheduling for nursing staff and visits daily -
Review orders, input and scheduling of nursing staff -
Assist with the referral approval process -
Participate in admin and/or clinical on call rotation
Coordination of patient telehealth monitoring systems
Fax discharge summary and transfer summary reports to physicians,
Assisted Living Facilities, and In-patient facilities for
coordination of care
Follow up with hospitalized patients
Assist with patient clinical care on an as needed basis
Administrative:
Assist with billing audits
Conduct wound care chart audits for accuracy and compliance -
Communications with clinicians, physicians and communities -
Follow up with physicians to received reporting and verbal orders
for referrals and current clients -
Cross train to become familiar with the intake process to provide
support -

Education:
Serve as the nurse educator and mentor for all new clinicians
ensuring they have - a successful onboarding process -

Job Type: Full-time
Required license or certification: LPN or RN
Required experience: Two (2) year of experience as a Nurse and One
(1) year experience in Home Health care required

Salary: $25.00 - $35.00 per hour

before starting work (Preferred)
Experience:
Unencumbered nursing license in the State of Oklahoma -
Knowledge of state and federal Home Health regulations
Excellent interpersonal, verbal and written communication
Valid driver's license, current auto insurance and reliable
transportation - -
Microsoft office experience -

Minimum of LPN/LVN (Required)
Work Location: Mainly one location
