These services address behavioral healthcare, physical healthcare, and social needs that are recovery focused, strengths based, culturally competent, patient centered, trauma informed, and co-occurring capable.Carry a differentiated caseload involving highly complex cases requiring intensive clinical service and extensive professional diagnostic and treatment experience. Provide crisis services, individual therapy, family therapy, group therapy and care coordination utilizing evidence based practices.Perform intake interviews and initial assessments, which includes the gathering of information about the patient's symptoms, behavior, functioning, history, needs, goals and preferences. Obtain necessary releases and consents and advise the patient about their rights and responsibilities. Consult with other program staff both within and outside of the agency to facilitate referrals.Work as part of a multi-disciplinary team to support change. Interact with the patients, their families, support system and community providers in a respectful, genuine and professional mannerObtain and maintain required credentialing including but not limited to licensure or under supervision status and Alcohol and Other Drugs Treatment Practitioner (AODTP) status. Obtain and maintain understanding and competency in any other instruments required by agency payors and/or commonly used by mental health/substance use professionals.Provide crisis management which may include face-to-face or telephone interviewing of the patient; assessment of a patient's lethality to self or others; assessment of a patient's available support systems; and assistance in arranging for appropriate intervention, including hospitalization. In the cases where a potential for harm exists, will complete appropriate assessment tools and, if needed and in consultation with management will facilitate referral to appropriate level of care. Immediately seeks consultation with supervisors regarding crisis situations, including potential safety concerns.Complete ongoing clinical documentation within approved time-deadlines. Respond in a timely manner to specific agency and clinical documentation requests within established deadlines. Returns phone calls and emails promptly.Participate in staff consultations for the purpose of case presentation and diagnostic and treatment planning for patients. Participates in mandatory program meetings.Maintain professional competency in his/her discipline through appropriate professional clinical trainings, attendance at workshops and conferences and via consultation with other professionals. Employee will comply with all requirements to maintain professional licensure. Must demonstrate good interpersonal skills and the ability to effectively communicate verbally with English-speaking patients. Must be able to move about facilities freely. Employee must have excellent written and oral communication skills. Casual professional dress is required with adherence to dress code requirements.Must have access to reliable transportation and travel is required. Must possess and provide verification of valid Oklahoma driver's license during employment with NorthCare. Must maintain and provide verification of current automobile liability insurance including uninsured motorist coverage. Must possess and maintain CPR, First Aid and CAPE certifications; or obtain within 30 days of employment with NorthCare. Must have current (within past year) TB test; or obtain within 30 days of employment with NorthCare.
