The RN Case Manager coordinates Hospice care services with the physician and interdisciplinary team and supervises the LPN and Home Health aides. He or she provides direct care and patient assessments and rotates in on-call coverage.

The ideal candidate is a graduate of an accredited school of nursing, is licensed as an RN in this state, and has one to two years of recent nursing experience. At least one year of home health care experience is preferred.$1,500 Sign-on bonus for all Full-time RN applications. Sign-on bonus will be paid out as follows:if you are hired for a full-time position, you will be paid $375 on your first paycheck.If you complete 60 days, you will receive a sign-on bonus of $375.If you complete 120 days, you will receive a sign-on bonus of $750, for a total of $1,500.

(Employees who have given notice or terminated at any point during this period are ineligible for this incentive program.)RESPONSIBILITIES

  • • Performs initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living
  • • Coordinates and implements the individualized plan of care recommending revisions to the plan as necessary
  • • Consults and educates the patient and family/caregiver regarding disease process, self-care techniques, end-of-life care
  • • Initiates appropriate preventative and rehabilitative nursing procedures
  • • Prepares clinical and progress notes that demonstrate progress toward established goal(s)
  • • Coordinates all patient and family/caregiver services and prioritization of needs with members of the interdisciplinary group
  • • Coordinates all patient and family/caregiver services and prioritization of needs with members of the interdisciplinary group and/or RN Case Manager
  • • Informs the physician and other personnel of changes in the patient's needs and outcomes of interventions
  • • Supervises LPNs and paraprofessionals providing services to the patient according to regulatory guidelines
  • • Participates in in-service programs or provides evidence of acceptable in-service received from another employer
  • • Perform basic nursing assessments/interventions on Home Health and/or Hospice care as directed by RN Case Manager.QUALIFICATIONS
    • Current state license as a Registered Nurse
    • One to two years of progressive nursing experience
    • Understands the hospice and home health philosophy of care
    • Current CPR/First Aid certification through the American Heart Association
    • Communication and interpersonal skills to develop relationships with clients, staff, and providers
    • Must be able to occasionally exert 50 to 100 pounds of force
    • Current driver's license and proof of automobile insurance in this state
    • Must have reliable transportation
